Once you've created a Form Template, it's easy to make a copy (i.e. make a template of a template). From the Team Templates page, select the [ ⁝ ] ellipsis button on the top right of the template that you would like to copy, then select Copy Template.

A popup modal will display, prompting you to create a name for the copied template.

Additionally, there is a checkbox that reads Copy responses from original template. When checked, copying the template will also copy any answers, notes, and questions marked not applicable. You probably want this checked.

Select the Copy Template button. A success notification will display in the top right, notifying you that the template was successfully copied. If this was an error, there is a button to Undo, which will remove the template.
