The intended audience for this article is Agents.

While on a personal or commercial Risk Profile, navigate to the Forms page.

Select the Actions button on the far right of the form that you would like to copy, then select Make a Copy.

A popup modal will display, prompting you to create a name for the copied form.

Additionally, there is a checkbox that reads Copy responses from original form. When checked, copying the form will also copy any answers and notes. This is useful if you need a copy of the form for a renewal.

Once you are satisfied with your form name, select the Copy Form button. A success toast notification will display in the top right, confirming that the form has been copied. Additionally, the copied form tile will be highlighted.
