Adds an option to unassign both the entire form and individual questions from a user on the forms index and forms response pages. This gives users better control over question and form assignments.

While viewing a form, users can choose to either Assign Selected Questions or Send Entire Form, giving users more granular control over what gets assigned.

When assigning a form by copying a link and not sending an email, the toast notification incorrectly stated that a form was emailed when only a link was copied.

In notification emails, if a segment doesn't have a logo or display name, use the team logo or display name instead. This applies to agency teams that have segments enabled and for agents or customers that belong to only one segment. If an agent belongs to more than one segment, that agent will always see the team branding.

Only the latest version of a published form should be available in the Forms Library.

Fixes an issue when two collaborators are assigned to a signature file. In some cases, packets were incorrectly marked as complete before all users signed.
